Before and after you get your vaccine

Continue to practice social distancing.  Wear a mask.  Wash your hands.  Getting the vaccine will help protect you from the virus, but maintaining these protective measures will still be necessary.

We have forms for the PDDH vaccine registration and consent forms you will need AFTER you make the appointment and BEFORE you go to the appointment. Call or stop by for no-contact pick up.

If we can help you in getting information, registering for VAMS or an appointment, please call us at (203) 263-2828.  We are here to help.

We also have cloth face masks ( in a set of 5) and liquid  hand soap ( in a set of 4)  available for free for Woodbury residents.   Call to reserve yours and make pick up arrangements.
